Small Business Intake Checklist

Business basics

• Legal business name, EIN, and entity type, including LLC, S-Corp, or other structure
• Business address and primary contact
• Prior-year business return, if available

Financial access, as applicable

• Monthly bank statements
• Credit card statements
• Accounting system access or export, such as QuickBooks

Revenue & expenses

• Sales/revenue reports, POS reports, or invoicing exports
• Major vendor bills and recurring costs
• Payroll records, if applicable

Operational details

• Contractors paid and 1099 needs
• Inventory details, if applicable
• Equipment purchases and asset records
